If your son or daughter is planning on participating in Athletics this season for Baldwin School, and our team is scheduled to compete vs Antilles Middle School and/or High School on the grounds of Ft. Buchanan, you are required to fill out the attached form DS 369.
The line items needed to be filled in BLACK ink ONLY are 2 through 8 & 11. The form must be submitted to Dirk Moran ASAP, so that he may send it in to the base. Sports that may compete on base at Buchanan are:

Boys Mini Basketball
Boys & Girls Mini Soccer
Mini Baseball
Varsity Baseball
Varsity Golf
JV Boys & Girls FUTSAL
