The 2026-2027 Student Health Records and updates must be uploaded and completed on the Magnus Health Portal account, starting June 23rd, with a completion DEADLINE of September 15th, 2026.

Requirements per grade are as follows:

• Physician Examination Form - All students
• Vision Exam Form - Students in PPK-PK-K-1-3
• Audiology Exam Form - Students in PPK-PK-K-1-3
• Dental Health Certification Form - Students in K-2-4-6-8-10
• Upload the updated PVAC-3 immunization record if the student is administered an additional new vaccine. The original paper hard copy (green form) must be delivered to the Infirmary.    
• For vaccine exceptions, upload a PR Health Department Medical or Religious Exemption Form to Magnus. Per the PR Health Dept., either form must be submitted annually. The original copy must be delivered to the Infirmary.

Books and Supplies

Librería Contemporánea has most of Baldwin School books and materials available throughout the year for sale on their website or in person at one of their locations. Please download the list for the school materials and books your child(ren) will need for the upcoming academic year.  The lists for the MUS contain direct links to the websites where specific digital books are to be purchased.

Student Handbooks pertaining to your child's school level are located in the Veracross Parent Portal account under the Parent Resource Links.
