NEW Student Health Record Requirements

Yearly NEW Student Health Requirements and Magnus Health Portal account.

The 2024-2025 Student Health Records must be uploaded and completed STARTING JULY 1st with a DEADLINE OF AUGUST 1st 2024. Secure access to the Magnus Health Portal for newly enrolled families will be provided on July 1st. 

We strongly recommend that parents schedule the physician appointments promptly before the July 1st opening of Magnus.  Download and Print the following forms HERE.

Requirements:

• Physician Examination Form - All students
• Vision Exam Form - Students from PPK- 3rd grade.
• Audiology Exam Form- - Students from PPK- 3rd grade.
• Dental Health Certification Form - grades K-2-4-6-8-10
• Updated PR Health Department Immunization record- PVAC-3 - All grades

       ** The original PVAC3 green form or PR Health Department's Medical or Religious  

             Immunization Exemption form must be delivered to the Infirmary every year.
